A consultation fee of $100.00 will be charged when a consultation appointment is scheduled. If you need to cancel or reschedule your appointment, SIAC requires two (2) business days notice to receive a refund. A consultation may be rescheduled only one time if 2 business days notice is not given before rescheduling.

As patients approach surgery, they frequently need information about the various payment options and have questions about their potential insurance benefits. We hope the following information will be helpful.

Payment for cosmetic and reconstructive plastic surgery is due in full at the time of your preoperative visit (2 weeks prior to surgery). We provide a number of payment options which may be used individually or combined according to your wishes. We accept personal check, cashier’s check, certified check, Visa, MasterCard and cash. We will be happy to assist you with applying for financing should you so desire.

Plastic & Reconstructive Surgery of Annapolis currently participates with Medicare & Informed Insurance Companies. For Out-Of-Network benefits we would be more than happy to help you submit to your insurance company. The Out-Of- Network benefits paid by insurance companies for plastic surgery varies greatly from carrier to carrier and plan to plan. Therefore, we make every effort to determine in advance if insurance coverage exists. We ascertain the projected insurance payment and the required co-payment. We do this because we believe you need to be as informed as possible before surgery. We know you realize that you are ultimately responsible for the full payment of your account, but we have found that our knowledge and experience can be an important factor in assisting you to collect your maximum benefits. Please discuss all arrangements regarding payment of your account with us.

We understand that a situation may arise that could force you to postpone your surgery. Please understand that such changes affect not only your surgeon but other patients as well. Dr. Sullivan’s time, as well as that of the operating room staff, is a precious commodity, and we request your courtesy and concern. Surgeries that are cancelled within 2 weeks of surgery are not eligible for a refund. A non-refundable deposit of a $1,000 is required to reserve a surgery date. This fee is NON-REFUNDABLE if surgery is cancelled or changed within 30 days of surgery date. If surgery proceeds as scheduled, the $1,000 fee will be applied towards the total surgeon’s fee. Final surgical payment will be due at your scheduled pre-operative visit or no later than two weeks prior to your scheduled surgery date. Your surgery will be cancelled if payment is not received by the due date. There will be a $35 returned check fee for any bounced checks. Fees for pathology, laboratory, radiology or any other outside services provided are not included in your quotes and are your responsibility. As prescription medications vary from patient to patient, they are a separate expense and are not included in this quote.

Due to the individual needs and requirements of each patient during surgery, the exact cost for the facility and anesthesia are not determined until after surgery. Responsibility for payment of the facility and anesthesia remains between the patient and the operative facility. Anesthesia contact is Lois Nolan 410-280-2260. This is an estimate only. Should you experience a complication requiring additional care and/or hospitalization, additional charges may result for which you will be responsible. We provide estimates of anesthesia and facility fees for our cosmetic enhancement patients as a courtesy for financial planning.

Skin Care Products are Non-Refundable unless you have an allergic reaction to the product. Dr.Sullivan will need to document the reaction with photographs in order to return the product back to the company.

Insurance

Dr. Sullivan offers her professional services to patients regardless of their insurance company or plan. Insurance is typically not involved for purely cosmetic procedures since they are not covered under most plans.

If your insurance coverage is limited to in-network providers, it will pay only for those physicians who are under contract to your insurance company, and this make exclude Dr. Sullivan. This does not mean you cannot choose Dr. Sullivan to perform your procedures – only that you will be responsible for payment.

If you have out of network benefits through your insurance plan we will be happy to help you submit the necessary forms for reimbursement of your medical costs. The amount you can expect to receive varies greatly among carriers and plans and will depend on your specific policy.
