Cosmetic/Self-Pay
If you are seeking treatment for a cosmetic procedure or something that is considered elective or self-pay, we will collect a $100 consultation fee at the time of your initial appointment. This fee covers the cost of the time and advice that you receive from the surgeon and is non-refundable. At the end of your consultation, a member of our billing team will discuss surgical fees and payment options available to you. The consultation fee does not go toward treatment.
If you decide to book surgery with us, a non-refundable deposit of $1,000 is required to reserve a surgery date, this deposit does go toward the cost of your surgery. We understand that a situation may arise that could force you to postpone or cancel your surgery. However, Sullivan Surgery & Spa does incur costs associated with scheduling your surgery; therefore the $1,000 deposit is non-refundable once surgery is booked.
Final surgical payment will be due at your scheduled pre-operative visit or no later than three weeks prior to your scheduled surgery date. Your payments will be broken down into three separate payments; to the Surgery Center, the Anesthesiologist and to Dr. Kelly Sullivan.Your surgery will be cancelled if payment is not received by the due date. Surgeries cancelled within two weeks of your surgical date are not eligible for a refund of surgical fees.

There will be a $50 returned check fee for any bounced checks.
Insurance Policy
Sullivan Surgery & Spa does not participate with private insurance plans (Blue Cross/Shield, Aetna, United Healthcare, Cigna, etc.). This is known as being “out-of-network”, we do accept Medicare. Often, our non-participation in private insurance is incorrectly simplified as meaning, “SSS doesn’t take my insurance”. What non-participation actually means is that SSS does not allow insurance companies to arbitrarily set coverage and payment for our services, and we do not necessarily accept their reimbursement as payment in full. The majority of private insurance plans have out-of-network coverage and will make payment for services rendered by non-participating providers. In fact, those plans typically charge additional premium for this out-of-network coverage (you are paying for this coverage; you should be able to use it).
We are more than happy to help you submit the necessary information to your insurance company to receive benefits to which you may be entitled, but we will not pre-authorize any surgical procedures. Pre-Authorization for surgical procedures is the patients responsibility. The benefits paid by insurance companies for plastic surgery vary greatly from carrier to carrier and plan to plan. It is the patients responsibility to determine in advance if insurance coverage exists, and what amount of “out-of-pocket” expenses you may be responsible for paying. After your consultation with Dr. Kelly Sullivan, a member of our billing team will discuss the surgical fees and payment options available for your procedures.
Dr. Sullivan offers her professional services to patients regardless of their insurance company or plan. Insurance is typically not involved for purely cosmetic procedures since they are not covered under most plans. If your insurance coverage is limited to in-network providers, it will pay only for those physicians who are under contract to your insurance company, and this may exclude Dr. Sullivan. This does not mean you cannot choose our doctor to perform your procedures – only that you will be responsible for payment.
If you have out-of-network benefits through your insurance plan, we will be happy to help you submit the necessary forms after surgery so you can be reimbursed for the medical costs, again our staff does not pre-authorize surgeries this is the responsibility of the patient. The amount you can expect to receive varies greatly among carriers and plans and will depend on your specific policy.
MedSpa & Skin Care Products
All Memberships, gift certificates and treatments performed at Sullivan Surgery & Spa are considered non-refundable. If you have an allergic reaction to the product. Dr. Sullivan and our Medical Spa Aestheticians will need to document the reaction with photographs in order to return the product back to the company. Med-Spa Memberships are also non- transferable.
Cancellation Policy
Should you need to cancel, please do so at least 24 hours in advance of your scheduled appointment. We understand emergencies and unforeseen circumstances may arise, please contact our office as soon as you can. If you cancel with less than 24 hours’ notice more than twice, your credit card on file will be charged the cancellation fee. Appointments scheduled for Monday must be canceled/rescheduled by 3pm on Friday in order to avoid cancellation fees. Should you arrive late for a scheduled appointment and time does not allow us to perform some or all of your treatment, the full amount of the scheduled service will be charged to you. The cancellation fee for all services is $50.
A copy of our Financial Policy is provided to all patients as part of their new patient paperwork. Additional copies are available upon request.
